This unique little item was purchased with the larger Bohemian garnet garnet brooch and stick pin which I have listed separately.  I was told that this is a "button cover" or shirt stud for a man's shirt.
This piece is a flower shape with lots of sizes of rose cut natural garnets. I see at least two missing garnets (see photos) and if you collect these, this (since there is only one of these button covers) would be a great piece to keep for making repairs to other Bohemian garnet pieces if that floats your boat.
This very old Bohemian garnet silver button cover's head measures 5/8" in diameter.  It weighs 2.7 grams and is in good antique condition other than the two missing garnets mentioned above.
