MDC 5-Way High Vacuum Fitting CF 2 3/4" Flanges
The fitting with flanges appears to be 5" length end to end & 5th tube hole is about 1 3/4" long. Two flange are stationary and three are rotatable. Marked MDC with no number. I took pictures with measurements to help.  I am not an expert and apologize the description is not more detailed.  I have a very similar unmarked version up as well. This came from a UC Berkeley lab surplus.  The nuclear science program is excellent.
